Description: Potassium Sparing Diuretic; Mineralocorticoid (Aldosterone) Receptor Antagonist

Spironolactone, Aldactone competitively inhibits aldosterone receptors on distal tubule of nephron (functional unit of the kidney) resulting in increased sodium, chloride, and water excretion whilst preserving potassium and hydrogen ions.  May block the effects of aldosterone on arteriole smooth muscle.  Has some antiandrogen (testosterone) activity and some estrogen activity.

Spironolactone, Aldactone is officially indicated for ascites (fluid buildup in abdomen from liver cirrhosis), heart failure with reduced ejection fraction, hypertension (high blood pressure), or primary aldosteronism.  It is also commonly prescribed off-label for acne vulgaris in women, heart failure with preserved ejection fraction, hirsutism in women (excessive body hair), hormone therapy for transgender females (male to female). (may be used for conditions not listed)

Before starting Spironolactone, Aldactone make sure your physician is aware of any allergies, medications you currently take, if you have liver disease, kidney disease, or are pregnant or breastfeeding.  This medication if given in excessive doses can result in significant diuresis which can lead to fluid and electrolyte depletion so patients must be monitored closely.

12.5-400 mg daily depending on condition divided into one or two daily doses

How to Take: Follow instructions provided by physician/pharmacy label, take consistently at same times each day unless otherwise directed, store at room temperature , ideally taken early in the day to avoid nocturia (waking up to urinate).  Take with or without food but be consistent to minimize fluctuations in drug exposure as food increases absorption.

You Need to Avoid: Do not skip follow up appointments or put off bloodwork requisitioned by your physician.  Serious electrolyte disturbances may occur without proper monitoring.

Gynecomastia (growth of breast tissue in men), hair loss, orthostatic hypotension, urinary frequency, dizziness, nausea, vomiting, muscle cramps, electrolyte disturbances, photosensitivity, stomach upset

1. Why do I have to go to the bathroom so often on Spironolactone, Aldactone?
   This medication helps with edema by getting rid of excess fluid, the way our bodies can rid itself of fluid quickly is through urination.  If you are urinating frequently then the medication is working.  It should be taken in the morning to avoid sleep disturbances from this issue.
2. I am already on a diuretic, why would my physician prescribe Spironolactone, Aldactone?
   It is common to add this medication to another diuretic as many other diuretics can cause rapid loss of potassium, so this medication reduces that effect.
3. What do I do if I show signs of high potassium levels (nausea, weakness, irregular heartbeat, tingling sensation, loss of movement)?
   Seek immediate medical attention, it may be a rare but potentially serious side effect
4. What do I do as a male if I notice my breast tissue is growing?
   Generally, this side effect is reversible with discontinuation of the medication, however in some scenarios the medication cannot be stopped immediately or needs to be replaced so make sure to contact your physician as soon as possible to discuss your options.
