Chlorhexidine/Periogard/Peridex is a positively charged molecule that binds negatively charged bacterial cell walls and extramicrobial complexes.  If the concentration of drug is low, then this throws off the osmotic equilibrium of the cell causing leakage of potassium and phosphorus resulting in halted growth/replication.  If the concentration is high, then cytoplasmic contents will precipitate resulting directly in cell death.   Chlorhexidine is broad spectrum and has activity against gram positive, gram negative, anaerobic, and aerobic bacteria, as well as yeast.

Chlorhexidine/Periogard/Peridex is officially indicated for gingivitis but it is also prescribed off-label for oropharyngeal decontamination for cardiac surgery patient or mechanically ventilated patients.   

Before starting Chlorhexidine/Periogard/Peridex make sure your physician is aware of any allergies, medications you currently take, if you are pregnant, or breastfeeding.

Swish 15mL (one tablespoon or capful) for 30-60 seconds after toothbrushing then spit twice a day in the morning and evening

How to Take: Follow instructions provided by physician/pharmacy label, store at room temperature.

You Need to Avoid: Do not rinse with water, other mouthwash, brush teeth or eat immediately after using chlorhexidine rinse as it needs time for its full effect.  Do not swallow.

Toothache, upper respiratory tract infection, sinusitis, taste alteration, tooth staining (long term use, usually removable by routine professional dental cleanings)
