Hypersensitivity/Anaphylactic reactions:
Serious and fatal hypersensitivity adverse reactions, including anaphylaxis, can occur with oxaliplatin within minutes of administration and during any cycle. Oxaliplatin is contraindicated in patients with hypersensitivity reactions to oxaliplatin and other platinum-based drugs. Immediately and permanently discontinue oxaliplatin for hypersensitivity reactions and administer appropriate treatment for management of the hypersensitivity reaction.”

Oxaliplatin, Eloxatin is a platinum derivative that works as an alkylating agent. Following intracellular hydrolysis, the platinum compound binds to DNA forming cross-links thereby halting DNA replication and transcription, resulting in cell death. Cytotoxicity is cell-cycle nonspecific.

Colorectal cancer; Colon cancer, stage III

Before starting Oxaliplatin, Eloxatin make sure your physician is aware of any allergies or medications you currently take, if you have renal impairment, are pregnant, or breastfeeding. Oxaliplatin, Eloxatin use has been associated with rare but serious adverse events such as bone marrow suppression, cardiotoxicity, irritation from extravasation, GI toxicity, hemorrhage, hepatotoxicity, hypersensitivity reactions, neuropathy, posterior reversible encephalopathy syndrome, pulmonary toxicity, and rhabdomyolysis.

IV: 85 mg/m2 every 2 weeks until disease progression or unacceptable toxicity (in combination with infusional fluorouracil/leucovorin).

How to Take: Administer as IV infusion over 2 hours; extend infusion time to 6 hours for acute toxicities.

Abdominal pain, anorexia, constipation, diarrhea, nausea, stomatitis, vomiting, anemia, leukopenia, thrombocytopenia, increased serum alanine aminotransferase, increased serum aspartate aminotransferase, increased serum bilirubin, fatigue, headache, insomnia, pain, peripheral neuropathy, back pain, cough, dyspnea, fever

1. Why would my physician prescribe a medication with such serious side effects?
   Your physician takes the most recent clinical guidelines to make evidence-based decisions on what medication is appropriate in which situations.  Your current medications, your allergies, age, cost of medication, comorbid conditions and many other factors will further complicate or limit which medications are appropriate.  This medication is used for certain forms of cancer and your physician has determined the benefits outweigh the risks.   Furthermore, they will be monitoring your bloodwork and symptoms closely to avoid many of the serious side effects and will keep you on the lowest effective dosage for the shortest amount of time possible.
