Gemfibrozil, Lopid reduces lipolysis and hepatic uptake of fatty acids along with secretion of VLDL cholesterol.  Gemfibrozil, Lopid also has been shown to increase HDL cholesterol; however the mechanism behind the HDL elevation is currently not known.

Hypertriglyceridemia in Fredrickson types IV and V hyperlipidemia

Before starting Gemfibrozil, Lopid make sure your physician is aware of any allergies or medications you currently take.  Gemfibrozil, Lopid can cause cholelithiasis, elevate hepatic enzymes, hematologic disturbances (i.e. anemia, leukopenia, thrombocytopenia, bone marrow hypoplasia), malignancy, myopathy/rhabdomyolysis.

Oral: 600mg twice daily.

How to Take: Take 30 minutes prior to meals.

Dyspepsia, atrial fibrillation, fatigue, vertigo, eczema, skin rash, abdominal pain, nausea, vomiting.

Note this is not a complete list of side effects for Gemfibrozil, Lopid only common ones.

1. What do I do if side effects are intolerable?
   If side effects are causing a lot of distress speak to your physician as soon as possible to discuss options.
2. Is this medication a good alternative to a statin?
   This class of medications has not demonstrated significant efficacy in reducing cardiovascular mortality in major studies unlike the statins.  They are effective at optimizing cholesterol lab values but have not shown meaningful reductions in heart attacks, strokes, etc., so statins are still preferred when they can be taken.
