Eplerenone, Inspra binds to the mineralocorticoid receptor and blocks the binding of aldosterone, a component of the renin-angiotensin-aldosterone-system (RAAS).  Aldosterone, a mineralocorticoid, increases blood pressure primarily by inducing sodium and water retention. Overexpression of aldosterone is thought to contribute to myocardial fibrosis (especially following myocardial infarction) and vascular fibrosis. Mineralocorticoid receptors are located in the kidney, heart, blood vessels, and brain. Eplerenone, Inspra selectively blocks mineralocorticoid receptors reducing blood pressure in a dose-dependent manner and appears to prevent myocardial and vascular fibrosis.

Hypertension, Post Myocardial Infarction with Reduced Ejection Fraction

Before starting Eplerenone, Inspra make sure your physician is aware of any allergies or medications you currently take.  Eplerenone, Inspra can cause hyperkalemia which is increased with renal impairment, proteinuria, diabetes, and patients taking concomitant angiotensin-converting enzyme (ACE) inhibitors, angiotensin-II receptor blockers (ARBs), nonsteroidal anti-inflammatory drugs (NSAIDs), and/or moderate CYP3A inhibitors. As a result, when taking Eplerenone, Inspra  serum potassium levels should be closely monitored. Eplerenone, Inspra should be used with caution in patients with pre-existing diabetes, heart failure and hepatic and/or renal impairment.

Oral: 25mg to 50mg daily.

How to Take: Can be taken with or without food.

Hyperkalemia, hypertriglyceridemia, dizziness, fatigue, hyponatremia, gynecomastia, hypercholesterolemia, diarrhea, abdominal pain, cough.

1. Why do I have to go to the bathroom so often on Eplerenone, Inspra?
   This medication helps with edema by getting rid of excess fluid, the way our bodies can rid itself of fluid quickly is through urination.  If you are urinating frequently then the medication is working.  It should be taken in the morning to avoid sleep disturbances from this issue.
2. What do I do if I show signs of high potassium levels (nausea, weakness, irregular heartbeat, tingling sensation, loss of movement)
   Seek immediate medical attention, it is a rare but potentially serious side effect of this
